A supervisor signature is required for this form to be considered valid. Valid submission of this form will help the Carson College of Business student complete required components of the Carson Career Amplifier Program.

This form will be evaluated to ensure the aforementioned activity meets the Carson College of Business guidelines for a High Impact Learning Experience. Upon approval, this form will be returned to the student signed.
